Eligibility Form

The following form contains two parts.

Part 1 is About Your Organization, where we will collect details about the following:

Organization name, address, EIN, and website URL;
Executive Director name and contact information;
Year established, organization budget, number of staff and volunteers;
Mission statement, description of organization, and population served; and
Copy of IRS 501(c)(3) determination letter, 990 IRS filing, and/or most recent financial statements.

Part 2 is the Grant Request, where we will collect details about the following:

Name of program/project the grant will help fund;
Requested grant amount;
Pillar of support (mental health, substance abuse, education, etc.);
Total amount budgeted for the program/project;
Description of why the grant is being requested;
How DJCFF's donation will be recognized.
